1995년에 정신보건법이 만들어지면서 만성 정신질환자를 치료관리하기 위한 체계적인 정책이 수립된다. 이를 계기로 정신질환의 조기발견과 정신과 치료를 강조하는 정신보건의료계의 노력이 계속된다. 이 글에서는 현재 정신보건의료의 영향력이 급속히 확장되고 있는 배경을 짚어보고, 정신적인 질병의 "치료화" 추세를 비판적으로 분석한다. 그리고 사회적 수준과 개인적 수준을 연결하기 위해서 생애사 연구방법 가운데 자전적 내러티브 분석을 적용한다. 질병 내러티브에 따르면 정신질환자는 초기에 생의학적으로 규정된 환자로서 사회화된다. 하지만 병원 시설에서 겪은 부정적 체험 때문에 오히려 초기사회화 과정에서 얻은 환자로서의 정체성에 거리를 두게 된다. 이와 반대로 사회에서는 병력 때문에 가족과 친구로부터 버림받게 된다. 이러한 모순된 상황에서 벗어나고자 정신질환자는 정상 생애사 단계에 재진입하려고 노력하나 좌절이 되풀이되고, 차츰 기능적인 뿐만 아니라 규범적인 일탈의 길로 접어든다. 하지만 만성 정신질환자들은 이에 대응하여 '질병공동체'를 통해서 하위문화를 형성하고 영웅적 냉소적 내러티브로서 자신들의 자아상을 보호하게 된다.

A systematic policy for treatment and management of chronic psychiatric patients in Korea has begun with the Mental Health Act(1995). Taking this opportunity, the psychiatric professions emphasize the significance of an early diagnosis and a psychiatric treatment. This paper discusses the background of the currently massive expansion of the psychiatric sector and criticizes the therapization of mental illness. For the linkage between the social level and individual level, biographic methods, concretely autobiographic narrative analysis are applied. According to the illness narratives, the psychiatric patients in the earliest stage are socialized as biomedical deviant. On the on hand, due to negative experience in the mental hospital and asylum they develop a new self-image, which helps them to keep distance to the other crazy patients. On the other hand, due to their medical history, their family and friends abandon them. Coping with the inconsistent situation, the psychiatric patients endeavor to reenter the normal biographic stage, often vainly and repeatedly, and finally they are heading toward functional and normative deviance. Under these circumstances chronic psychiatric patients build their subculture as a illness community. Besides, they tend to cope with their destiny by telling ironic and heroic narrative.
