  • 본 연구는 단면 역학연구로서 한국인의 골밀도를 미국의 백인, 흑인, 히스패닉, 아시아계, 그리고 카리브 연안의 토바고 흑인 및 홍콩 중국인 등과 비교하고 관련 요인을 파악하고자 실시되었다. 65-78세 남성의 대퇴골두, 골반부, 요추 부위 골밀도 비교를 위해 Osteoporotic Fractures in Men (MrOS) study(미국의 비히스패닉 백인 4074명, 흑인 208명, 히스패닉 116명, 그리고 아시아계 157명), Tobago Bone Health Study(카리브 흑인 422명), MrOS Hong Kong study(홍콩 중국인 1747명), 그리고 Namwon Study(한국인 1116명) 등으로부터 골밀도과 주요 골다공증 위험요인 변수들이 포함된 분석자료를 수집하였다. 골밀도 측정치는 모든 측정기들에 대해 상호 보정되었다. 인종군간의 골밀도 평균의 비교를 위해 다변인 일반선형회귀 모형을 적용하였으며, 미국 백인 골밀도로 부터 각군의 골밀도가 차이나는 정도를 백분율로 제시하였다.
    연령보정후 미국 백인과 비교시 한국인의 대퇴골두, 골반부, 요추 부위 골밀도는 각각 -15.4, -25.4 and -17.6% (p<0.001 for all)의 차이를 보였다. 토바고 흑인의 경우 미국 백인에 비해 대퇴골두, 골반부, 요추 부위 연령보정 골밀도가 각각 11.6 (p<0.001), 7.9 (p<0.001) and 1.0% (p>0.1) 높았으며, 미국 흑인의 경우도 각각 11.3, 7.3 and 6.1% (p<0.001 for all) 높았다. 미국 히스패닉의 경우 미국 백인에 비해 대퇴골두, 골반부, 요추 부위 연령보정 골밀도가 각각 1.8, 0.3 and -2.9% (p>0.05 for all), 미국 아시안의 경우 각각 -3.5 (p=0.055), -4.9 (p<0.001) and -2.9% (p>0.1)의 차이를 보였다. 홍콩 중국인의 경우 미국 백인에 비해 대퇴골두, 골반부, 요추 부위 연령보정 골밀도가 각각 -6.8, -7.5 and -10.1% (p<0.001 for all)로 한국인 다음으로 낮은 골밀도를 보였다. 신체크기를 나타내는 신장과 체중을 보정한 결과 미국 백인과 아시아인(한국인, 홍콩중국인, 미국 아시아계) 사이의 골밀도 차이가 현저히 감소하였으나 미국 백인과 한국인 간의 두드러진 차이는 여전하였다. 조사 당시의 높은 흡연 및 음주율, 낮은 칼슘 섭취량 등도 유의한 기여요인이었지만 한국인과 미국인의 골밀도 차이에 대한 기여도는 신장 및 체중의 기여도에 비해서는 매우 작았다. 조사대상 한국인들은 8-22세 기간에 한국전쟁을 경험하였는데, 이시기의 영양결핍등의 요인이 낮은 골밀도에 기여하였을 것으로 사료된다.

  • There is insufficient epidemiologic information about men’s bone mineral density (BMD) levels across ethnic groups and geographic locations. In a cross-sectional design, we compared older men’s mean BMD among seven ethnic groups in four countries. Femoral neck, total hip and lumbar spine BMD were measured in men (age 65 to 78 yrs) from the Osteoporotic Fractures in Men (MrOS) study (4074 Caucasian, 208 African American, 157 Asian and 116 Hispanic men in US), Tobago Bone Health Study (422 Afro-Caribbean men), MrOS Hong Kong study (1747 Hong Kong Chinese men) and Namwon Study (1116 South Korean men). BMDs were corrected according to the cross-site calibration results for all scanners. Least square means of BMD (LSMs) among ethnic groups were estimated by multivariable general linear models and the percentage differences in LSMs between US Caucasian men and other ethnic groups were calculated. All p-values to test these differences were based on Tukey-Kramer adjustment for multiple comparisons of LSMs.
    Compared with US Caucasian men, the percentage differences in the age adjusted mean BMDs at femoral neck, total hip and lumbar spine, respectively, among ethnic groups were 11.6 (p<0.001), 7.9 (p<0.001) and 1.0% (p>0.1) in Tobago Afro-Caribbean men; 11.3, 7.3 and 6.1% (p<0.001 for all) in African American men; 1.8, 0.3 and -2.9% (p>0.05 for all) in US Hispanic men; -3.5 (p=0.055), -4.9 (p<0.001) and -2.9% (p>0.1) in US Asian men; -6.8, -7.5 and -10.1% (p<0.001 for all) in Hong Kong Chinese men; and -15.4, -25.4 and -17.6% (p<0.001 for all) in South Korean men. Additional adjustment for weight and height greatly attenuated the differences between US Caucasian men vs. Asian ethnic groups such as US Asian, Hong Kong Chinese and South Korean men. Despite this attenuation, some differences, particularly those between South Korean and US Caucasian men, still persist.
    In conclusion, men of African origin had substantially higher hip BMD than US Caucasian men. US Hispanic, US Asian and Hong Kong Chinese men had similar hip BMD. South Korean men had the lowest hip and spine BMD even after adjusting for age, height and weight. These South Korean men were all age 8 to 22 yrs at the beginning of Korean War. Malnutrition and other factors may have influenced their peak skeletal mass.

  • 연구결과:한국 남성은 미국 Caucasian men, Hispanic, African origin에 비해 매우 낮은 골반부, 대퇴골 및 척추 골밀도를 보였다. 한국인의 이러한 낮은 골밀도는 홍콩 등 다른 아시아인과 마찬가지로 서구인에 비해 작은 신장과 체중 요인이 기여하는바가 컸다. 아울러 한국전쟁 등 열악한 사회경제적 수준으로 인한 영양부족등의 요인도 조사대상 한국인들의 낮은 골밀도에 기여하였을 것으로 사료된다. 높은 흡연율 및 음주율, 낮은 칼슘 섭취량 등도 한국인과 미국인의 골밀도 차이에 유의한 기여요인이었지만 신장 및 체중의 기여도에 비해서는 매우 작았다.
    활용방안: 골밀도는 골절예방을 위한 중요한 지표이다. 본 연구결과는 미국인에 비해 매우 낮은 한국인의 골밀도에 대한 관리의 필요성을 보여준다. 본 연구에서 제시한 미국의 여러 인종들의 평균 골밀도를 국가 골다공증 예방사업의 목표치 설정의 근거로 활용할 수 있을 것이다.
